This is an alphabetical listing of the more frequent plans accepted, but we accept other plans as well:
We ask you provide us with a copy of your insurance information before your examination at our offices during normal business hours.
If you provide us this card for an after hour or weekend examination it may not be possible to obtain coverage information until the next business day. We can do an insurance verification and under normal circumstances properly advise you of coverage.
