On Saturday March 18, 2017 Dr Spero Kinnas was one of the Guest Faculty presenting at the Northwestern University Cataract Symposium.
The symposium was put together to discuss the latest techniques and technological advances for the surgical management of cataracts.
Apart from guest faculty and professors from Chicagoland universities, in attendance were experts from the University of Utah, the Cincinnati Eye Institute and the University of Miami.
Treatment of Cataracts in Cases of Advanced Glaucoma
Dr. Kinnas presented his experience with femtosecond laser surgical treatment of cataracts in cases of advanced glaucoma that have been previously treated by a procedure called trabeculectomy.
Glaucoma is a vision threatening condition resulting from intraocular pressure damage to the optic nerve. The hallmark of trabeculectomy is the creation of a filtering bleb. A filtering bleb is a thin semi lucent membrane created so the excess fluid pressure within the eye can no longer harm the optic nerve.
Femtosecond Laser Cataract Surgery Advancements
For decades, traditional cataract surgery, without femtosecond laser, is known to result in dysfunction or failure of up to 50% of previously functioning filtering blebs.
With the introduction of femtosecond laser technology for the surgical care of cataracts, there is an opportunity to address cataract surgery patients who have had previous glaucoma filters with a more precise and less traumatic methodology.
The advantages of the femtosecond laser for cataract surgeries are that incisions can be applied without blades, and the cataract can be de-constructed prior to entry to the eye. If the cataract is deconstructed, less manipulation is required to complete the cataract surgery.
Less manipulation is a considerable advantage in limiting post-operative inflammation and scarring that could result in the dysfunction or failure of a glaucoma bleb.

When asked, Dr. Surendra Basti, the course director, stated Dr. Kinnas is one of two eye surgeons he knew that successfully performed femtosecond laser cataract surgery in a patient who had functioning glaucoma filtering blebs. The other is Dr. Mahipal S. Sachdev, M.D. of the Centre For Sight in New Delhi, India.
